Information Library for Families

We have developed an online library of information and materials for families and children with special health care needs. Some of these materials are specific for families and children served by DSCC programs, while others are general materials that may be of interest to any family or child with special health needs. Materials on this page are listed alphabetically.

We hope you find this information helpful. If you have suggestions or comments about the materials provided here, please use our Contact Us page to communicate with us and help us make this a beneficial resource.

Application Forms for DSCC Programs

If you are interested in applying for either the DSCC Core Program or the Home Care Program, you can print the following application forms and mail them to the DSCC office in your area. Use the Find an Office link on our web site to locate the office in your area. More information about each of these programs is available from our Home page.

You can fill-in and print completed forms from our web site, print blank forms and complete them manually, or contact a DSCC Regional Office to receive them by regular mail. The Adobe Reader, available free from the Adobe web site, is needed to view and print these PDF files.
