Information Library for Families
We have developed an online library of information and materials for
families and children with special health care needs. Some of these materials
are specific for families and children served by DSCC programs, while
others are general materials that may be of interest to any family or
child with special health needs. Materials on this page are listed alphabetically.
We hope you find this information helpful. If you have suggestions or
comments about the materials provided here, please use our Contact Us
page to communicate with us and help us make this a beneficial resource.
Application Forms for DSCC Programs
If you are interested in applying for either the DSCC Core Program
or the Home Care Program, you can print the following application forms
and mail them to the DSCC office in your area. Use the Find
an Office link on our web site to locate the office in your area.
More information about each of these programs is available from our Home
page.
You can fill-in and print completed
forms from our web site, print blank forms and complete them manually,
or contact a DSCC Regional Office to receive them by regular mail. The
Adobe Reader, available free from the Adobe
web site, is needed to view and print these PDF files.
Note: Adobe Reader does not allow you to save completed forms to disk;
the ability to save completed forms is available commercially with the
Adobe Acrobat 3.0 (or later) product suite.
You can also call your nearest DSCC office and request application forms.
You will be asked to provide financial and health care information about
your child and family. These forms are needed to begin the application
process.

Coordinated Care Record
This workbook was developed to help families record and organize information
for their child with special health care needs. It can also be used to
help bring together and coordinate the many services and providers of
care involved in the child's life.
The Coordinated Care Record is
also available in a single PDF file; you will need the Adobe Reader
to view and print this document. The Reader is available free from the
Adobe
web site. This is a large file (approx. 500KB) and may take several
minutes to download, depending upon your modem connection.

DSCC Brochures

- Institute for Parents of Preschool Children Who are Deaf or
Hard of Hearing
Provides information about the annual Institute held in June of each
year in Jacksonville, Illinois. This is a great opportunity for parents
and their children, age birth to 5, to spend a week, completely free
of charge, with experts in the field of deafness from all over the
state. The professionals’ job is to give parents the information
they need to help their children with hearing loss. Parents receive
informational materials, meet other parents, learn about teaching
language, hearing aids, cochlear implants, potty training, discipline
and more! Children receive evaluations and a report for each child
is sent to families after the Institute ends. Read the brochure to
see how to apply to attend. The brochure is available in both English
and Spanish.
- Newborn Hearing Screening in Illinois
The Illinois Department of Human Services (DHS), Illinois Department
of Public Health (IDPH) and The Division of Specialized Care for Children
(DSCC) have developed informational brochures for parents of children
who received the newborn hearing screening.
“Newborn Hearing Screening in Illinois,”
provides information on the importance of hearing screening and communication
milestones for infants. The brochure is available in English
(43.00), Spanish (43.00S) and Polish
(43.00P).
“Illinois Universal Newborn Hearing Screening Important
Instructions to Parents,” provides information to parents
on the process of obtaining a follow-up diagnostic services for the
infant after a referral. The brochure is available in English
(43.01), Spanish (43.01S) and Polish
(43.01P).
- DSCC and All Kids brochure
Information about how the Illinois All Kids program and the DSCC
program work together is now available in a brochure titled DSCC
& All Kids - Working Together.
- DSCC Family Handbook
The DSCC Family Handbook is available in both English
and Spanish versions
from our website.
The handbook includes answers to the most common questions families
have about DSCC services, and explains how to contact us for more information
or assistance. The DSCC Family Handbook can also be obtained at no cost
through our network of Regional Offices. See our Find an Office web

DSCC Family Newsletter
Our family newsletter, Special Addition, is published
twice a year (in both English and Spanish) and provides national and state
information for families of children and youth with special health care
needs in Illinois. Mr. Bob Cook, our Family Liaison Specialist, serves
as the editor of this newsletter. Please visit our Family

DSCC has developed the following material to assist families in understanding

Illinois
Provider Directory for Children with Disabilities
A statewide resource for locating pediatric health care providers
in Illinois and the surrounding border states is now available online.
Families can use the directory search features to locate providers by
the type of specialty needed, by the type of health care services needed,
or by the city or county where they want to obtain care.
Providers listed in the directory include pediatricians, family physicians,
pediatric specialists, occupational therapists, physical therapists, speech
pathologists, audiologist, pediatric dentists and others. To locate specialty
providers in your area, visit the Provider Directory for children with
disabilities.

Medical Documents of Interest
To learn more about health issues that your child may be facing, DSCC
has pre-defined a collection of journal articles on health conditions
and treatment available through the National Library of Medicine (NLM).
You can click here to let us help you. We
have provided a list of specific medical conditions for you to select
from. The NLM provides a brief summary or "abstract" of the
journal article. For a fee, you can obtain the complete article from any
library that carries the journal containing the article.
Another valuable resource provided by the National Library of Medicine
is MEDLINE plus. MEDLINEplus will
direct you to information to help answer health questions. MEDLINEplus
brings together, by health topic, authoritative information from NLM,
the National Institutes of Health (NIH), other government, non-profit
and other health-related organizations. Preformulated MEDLINE searches
are included in MEDLINEplus and offer easy access to the medical research
literature. This resource also provides you with a database of full-text
drug information and an illustrated medical encyclopedia.
