Coordinated
Care Record
The Coordinated Care Record was designed by a committee
of parents of children with special health care needs and DSCC staff
to help families record and organize information about their child's
needs and services. The pages can be easily maintained in a 3-ring binder
with divider tabs to separate the sections.
Below is an outline of the contents of the Coordinated Care Record with
hyperlinks to individual pages or sections of the workbook. This is provided
to make it easier and faster to view or print the information. The entire
workbook is also available as a single file. Click the Families button
on the navigation bar at the right to find out more information about
this option.
You will need the Adobe Acrobat Reader to view and print these
documents. The Reader is available free from the Adobe web
site.
