Forms and BrochuresIllinois Provider Directory for CSHCN: Application FormThe Illinois chapter of the American Academy of Pediatrics provides an online searchable directory of pediatric health care providers to help families and health care professionals find the care and services needed in their community. Emergency Medical Information FormThis form can be used to provide critical and important health care information that will assist emergency medical personnel to quickly and effectively meet a child's urgent health care needs. This form was developed by the American College of Emergency Physicians and the American Academy of Pediatrics. Illinois Pediatric Consultant ListThe UIC Division of Specialized Care for Children (DSCC) has identified the following pediatric specialists in Illinois who are available to primary care providers to answer general questions about the management of children with special needs. DSCC will reimburse primary care providers for telephone consultations with the consultants on the list regarding DSCC-approved clients using appropriate CPT codes. Medical Home Brochure for FamiliesDSCC has developed a brochure to introduce the medical home concept to families. It highlights the importance of having a partnership between parents and primary care providers to successfully coordinate care for children with special health care needs. Medical Home Brochure for PhysiciansDSCC has developed a brochure to introduce the medical home concept to physicians. It highlights the importance of having a partnership between parents and primary care providers to successfully coordinate care for children with special health care needs. Provider Notes/Family Survey FormDSCC developed this form to be used by Primary Care Physicians and families to summarize family issues as well as medical findings and recommendations. This form can also be used, with the family's consent, to share information with other health care providers involved in a child's care Transition Facts for Health ProfessionalsThe Illinois Chapter of the American Academy of Pediatrics (ICAAP) developed this brochure to guide health professionals in assisting teens with the issues faced in the successful transition into adulthood. Transition Issues for Adolescents & their FamiliesThis brochure developed by the Illinois Chapter of the American Academy of Pediatrics offers assistance for families with issues related to transition planning for adolescents with special needs. (This brochure is also available in Spanish: Asuntos de transición para adolescentes con necesidades especiales) PublicationsData Resource Center for Child & Adolescent HealthThe Data Resource Center for Child and Adolescent Health provides national, state, and regional data on the health and health-related services for children, youth and families in the United States. Data from both the National Survey of Children with Special Health Care Needs (CSHCN) and the National Survey of Children’s Health (NSCH) is detailed on this web site. Recommendations for Preventive Pediatric CareThis Policy statement from the AAP Committee on Practice and Ambulatory Medicine and Bright Futures Steering Committee offers recommendations for well child care and emphasizes the importance of continuity of care from the prenatal period through adolescence. Other ResourcesThe National Center of Medical Home Initiatives for Children with Special Needs is an extensive and valuable resource for all things medical home. In June 2009 the AAP/MCHB/National Center for Medical Home Implementation published the Building Your Medical Home Toolkit that supports primary care pediatricians' development and improvement of a Medical Home. It also prepares a pediatric office to apply for and potentially meet the National Committee for Quality Assurance (NCQA) Physician Practice Connections Patient Centered Medical Home (PPC-PCMH) Recognition program requirements.
