The role of the primary care physician in providing a medical home is to utilize critical supporting elements - basic knowledge, professionally endorsed policies, best practice models and community resources - while providing care that is accessible, coordinated, continuous, comprehensive, family-centered, compassionate and culturally competent.

The following topics, developed by the Utah Medhome Portal, will exemplify how the medical home model can be applied to the management of a specific chronic health condition in the primary care setting. Establishing a medical home ensures access to appropriate care and treatment and also provides for continuity of care. The content of the following topics may vary from state to state, or community to community, based on different resources or local policies.

The medical and service needs of children with special health care needs are often complex and diverse. Such children often require services from a variety of sources, over long periods of time and at high cost. The Physicians Guide to Caring for Children with Disabilities and Chronic Conditions, by Dr. Larry Desch and Dr. Robert E. Nickel, provides valuable information and condition-specific care coordination templates for managing the care of children with special health care needs.
